It is endemic to Hawaii, where it could previously be found on the islands of Molokaʻi and Oʻahu; today populations only exist on the latter. This is a federally listed endangered species of the United States.

Eugenia koolauensis is one of only two species of Eugenia native to Hawaii. The other is E. reinwardtiana, which occurs naturally in Hawaii as well as through much of the tropical Pacific Ocean. Eugenia reinwardtiana is a rare plant in Hawaii, except for parts ofthe northern Waianae Mountains where it can be fairly common.
